Piercing jewellery by placement: what to wear where

Every piercing has its own rules. The same stud that sits perfectly in a healed lobe will be the wrong gauge, the wrong length and the wrong shape for a helix, and in a fresh cartilage piercing the wrong size does not just look off, it slows healing down. This page is the shortcut: what actually goes in each spot on the ear, and why.

These are the usual starting points, not a prescription. Your anatomy and your swelling decide the final length, which is exactly why downsizing matters. Lobe and upper lobe

The lobe is soft tissue rather than cartilage, which is why it heals fastest and forgives the most. Healed lobes are usually 18g, though plenty of people wear 16g, and this is the one place where a thinner decorative earring is genuinely fine once everything has settled. For a fresh lobe, a flat back labret beats a butterfly back every time: nothing to dig in when you sleep, nothing to trap behind the ear. Helix and forward helix

The helix is the outer rim, and it is cartilage, so it is a different animal to the lobe. 16g is the standard. A flat back labret is the safest choice while it settles, and rings usually come in at 8mm, though the right diameter depends on how much rim you have. The forward helix sits at the front where the rim meets the head, and the tight space means a short post and a low-profile top. Flat

The flat is the broad, flat panel of cartilage inside the upper rim, and it is the most forgiving place on the ear to wear something with presence. 16g, flat back labret, and enough space that a larger decorative top will not press on anything. It is the placement people usually mean when they say they want one piece that gets noticed.

Rook and daith

The rook sits on the ridge above the inner bowl and takes a curved barbell rather than a straight post, because the tissue it passes through is folded. The daith is the small ridge just above the ear canal, and it is a ring placement: a clicker or a seam ring, not a stud. Both are 16g, both are cartilage, and both are slow. Conch

The conch is the bowl in the middle of the ear, and it is the one placement that genuinely works both ways. An inner conch takes a flat back labret; an outer conch takes a ring, usually 10mm or 12mm because it has to clear the whole bowl. 16g is standard, though some conch piercings are done at 14g for a sturdier bar. Tragus and anti-tragus

The tragus is the small flap in front of the ear canal. It takes 16g and a short flat back labret, and the flat back genuinely matters here, because anything protruding sits against the side of your head all day. The anti-tragus, the little ridge opposite, is one of the fussier placements on the ear: an awkward angle, a curved barbell or small ring, and a healing time that tests your patience.

Industrial

An industrial is not really one piercing, it is two helix piercings joined by a single straight barbell, usually at 14g or 16g. That is why it is harder than it looks: both piercings have to be placed so a straight bar can pass through them at a natural angle, and both have to heal while sharing one piece of jewellery that moves when either one moves.

What every placement has in common

  • The metal. Implant-grade titanium, ASTM F-136, for anything fresh or fussy. No exceptions, and no unmarked “surgical steel”.
  • The threading. Threadless or internally threaded, so no screw thread is ever dragged through the piercing channel. We explain the difference in threadless vs threaded.
  • The gauge. Match the piercing, never guess. A 20g fashion earring in a 16g cartilage piercing will move, tilt and irritate.
  • The downsize. Fresh piercings are fitted long to allow for swelling. Once that settles, a shorter post stops the piece catching and snagging.

What gauge is a helix piercing?

16g, almost always. 16g is the workhorse gauge for cartilage across the ear, including helix, conch, tragus, rook and daith. If you are unsure what your cartilage piece is, 16g is the most likely answer, but measure rather than assume.

What size hoop do I need for a helix?

Usually 8mm. Diameter depends on how much rim the ring has to clear, so a piercer fitting your ear will beat any chart, but 8mm is the standard starting point for a helix and 10mm or 12mm for a conch.

What jewellery goes in a conch piercing?

Either a flat back labret or a ring. An inner conch usually takes a labret; an outer conch usually takes a ring at 10mm or 12mm, because it has to clear the whole bowl. Both are typically 16g.

Can I wear a normal earring in a cartilage piercing?

Usually no, and gauge is the reason. Most fashion earrings are 20g, while cartilage piercings are typically 16g. A thinner post in a thicker hole moves around, sits crooked and irritates the channel, and butterfly backs press into cartilage in a way flat backs do not.
